PARISH CONSULTATION SURVEY - Fall 2024

As the Universal Church engages in the second phase of the Synod on Synodality, we are inviting members of Saint Patrick's Parish in Wildfield to express their thoughts and opinions on where we are as a parish community. We have prepared this questionnaire in hopes that it will be able to facilitate as many people as possible to be able to participate in this parish consultation.

You have three options to participate in this consultation/survey:

OPTION ONE

You may pick-up a copy of the questionnaire at the back of the church and fill it out. You may drop-off the completed questionnaire in the regular Sunday collection basket, or in the sacristy after Mass during the week.

OPTION TWO

You may download a copy of the questionnaire, and fill it out electronically. You may then e-mail the completed questionnaire to the parish office - or you can print out your responses and then drop it off in the regular Sunday collection basket, or in the sacristy after Mass during the week.

OPTION THREE

You may use the electronic version of the questionnaire below and submit your responses online.
